About the RoleKDC is seeking an experienced AFC Registered Nurse to provide clinical oversight and support for adults with medical needs and/or developmental disabilities in our Adult Foster Care program. Using a team-based, person-centered approach, you will conduct assessments, oversee care plans, support caregivers, and ensure regulatory compliance, all while promoting independence and high-quality care for participants.
Schedule and Compensation30-32 hours/week, Monday-Friday. $32-33/hour.
Required Qualifications- Current Massachusetts RN license.
- Minimum of 3 years of experience in human services, long-term supports, disability services, or related programs.
- Knowledge of home- and community-based service models, AFC, Shared Living, or Mass Health programs.
- Strong communication, organizational, and interpersonal skills.
- Proficiency with Electronic Health Records (EHR) and Microsoft Office.
- Valid driver’s license and reliable transportation.
- Ability to complete required trainings such as CPR and First Aid.
- Experience in community-based programs.
- Familiarity with Medicaid, Mass Health, or state funding systems.
- Bilingual or multilingual skills.
- Conduct clinical assessments, develop individualized care plans, and provide ongoing oversight of AFC placements.
- Perform monthly home visits and monitor participant health, responding promptly to changes.
- Provide guidance, education, and support to caregivers regarding medical concerns and wellness practices.
- Coordinate community medical services and collaborate with healthcare providers, caregivers, and internal teams.
- Maintain accurate documentation in the Electronic Health Record (iCentrix) and ensure compliance with agency and regulatory standards.
- Serve as a clinical resource for caregivers, case managers, and interdisciplinary team members.
