Be the first welcome our residents receive.
Moving into a care facility is a big step for residents and their families. As our Admissions Coordinator, you'll guide them through it with clarity and compassion. You'll also work behind the scenes with hospitals, our clinical team, and referral partners so every new resident arrives to a room, a care plan, and a team that's ready for them.
What You'll Do- Review incoming referrals and screen prospective residents to confirm we can meet their clinical needs
- Coordinate with hospitals and case managers on admissions, including complex cases
- Make sure the right equipment, supplies, and staffing are in place before each resident arrives
- Manage bed assignments, including cohorting and thoughtful roommate matching
- Gather personal, medical, and insurance information and complete all admission paperwork accurately
- Serve as a warm, reliable point of contact for residents and families throughout the process
- High school diploma or equivalent
- Long-term care experience required.
- Strong communication and people skills, with sound judgment around confidential information
