Position: Direct Support Professional (Citizens)

. Practice and promote the agency’s mission in every interaction and provide ongoing support to individuals who live in the Residential Services program to ensure a high quality of life is maintained. Support residents with self‑care routines, health, safety, household chores, socialization, and overall wellbeing.

Responsibilities
  • Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in all aspects of their lives. Offer only the level of support necessary for each person to promote independence.
  • Support residents to become active community members and to build and maintain natural supports and relationships.
  • Ensure a safe, clean home, assist with, or participate in, household chores as needed.
  • Follow treatment plans and use adaptive/safety equipment to promote independence and ensure health and safety.
  • Teach residents to prepare meals, follow diet adherence, portion control, and cooking skills.
  • Assist residents with medication administration according to the AMAP policy and procedures.
  • Implement and document all treatment plans and agency materials (residential habilitation goals, medical case notes, communication logs, etc.) to aid residents in achieving independence.
  • Adhere to overtime, scheduling, and minimum staffing policy to ensure quality support.
  • Participate in the inter‑disciplinary team; attend meetings to discuss progress and goals.
  • Provide transportation for medical appointments, shopping, recreational outings, and other travel needs.
  • Mentor new direct support staff; demonstrate supportive routines and review training manuals.
  • Participate in house meetings and maintain all certification and training expectations.
  • Perform any other duties as requested by the supervisor.
Qualifications
  • High school diploma or equivalent preferred.
  • Valid NYS driver’s license, with continuously qualified driver status.
  • Ability to communicate verbal and written instructions effectively.
  • Strong interpersonal and communication skills with a high level of empathy and patience.
  • Documentation of ongoing physical contact for this role is required; successful completion of a background check is mandatory.
  • Physical ability to lift and transfer individuals; the physical demands may impact employment if cannot be met.
Training & Requirements
  • Training on Personal Outcome Measures (POM) is required prior to completion of evaluations and assessments.
  • Background check and fingerprinting required due to unsupervised physical contact.

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services; therefore, a successful background check is required.
