Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule:

Full-time | Monday
- Friday

Shift: 11 AM- 7 PM or 3 PM- 11 PM

Location:

Toledo, Ohio and surrounding area

• Licensed Nursing Assistant
• CPR/BLS Certified
• Must be willing to travel
• Valid Drivers License and Auto Insurance Coverage
• Reliable transportation
• Hospice experience preferred

• Perform personal care for the patient, including bathing, dressing, toileting, and feeding, according to the hospice aide plan of care.
• Document care provided according to the Care 360 Hospice policy.
• Report any deviations from the plan of care to the case manager.
• Use durable medical equipment safely
• Take vital signs when issued as part of the plan of care
• Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
• Assist with ambulation, transfers, and range of motion if documented on the hospice aide plan of care.
• Participate in QAPI activities
• Participate in IDG meetings
