Description
The RN Weekend Supportive Care Nurse (day shift) provides one-to-one care at the bedside of terminally ill patients, based on an ongoing nursing assessment of need and in accordance with the established plan of care.
Duties- Provides compassionate end-of-life care in accordance with the individualized plan of care, with demonstrated ability to assess and respond to patient and family needs in a timely manner
- Educates patients and caregivers on end-of-life, services and other resources
- Documents complex medical information using clinical tools, data and other information to complete assessment of patient needs
- Establishes professional and collaborative relationships with patients, families and providers utilizing strong written and verbal communication skills
- Good computer skills, including working knowledge of the Microsoft Office suite
Day shift, part time (24 hours) or full time (36 hours): 8:00 am - 8:00 pm, Saturday and Sunday with weekend shift premium
Care Site - private residences and facilities in Kentucky and Ohio
RequirementsGraduate of an accredited School of Nursing and current licensure in Kentucky/Ohio. At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred. Personal qualities of empathy, responsibility and dependability. Must demonstrate effective communication skills with team members, patients, providers, and family members. Must have ability to work independent of direct on-site supervision.
Must arrange travel and be able to access client homes, nursing homes, hospitals, and other facilities independently. Must demonstrate proper use of personal protective equipment and observe universal precautions. Must be able to lift at least 50 pounds using proper body mechanics. Must have valid driver's license, current auto insurance and reliable transportation.
