Responsibilities
- Implement assigned outreach activities according to the community outreach plan, ensuring logistics for health screenings, fairs, education programs and special events are completed timely, efficiently and accurately.
- Represent the Community Outreach department on boards, coalitions and committees aligned with the community benefit plan, report meeting information and follow‑up items.
- Identify and maintain relationships with key community contacts that share the mission for improving health within the communities we serve.
- Coordinate nursing and health promotion services based on community need and assess populations at risk.
- Accurately collect and input data into outreach database; submit monthly management reports and provide program attendance, event summaries and evaluation forms per protocol.
- Coordinate with nurses, health educators, community members, physicians and other employees for scheduling health activities and organize presentations.
- Gather and share community health information, keep current with public health trends, and communicate effectively with professionals and organizations.
- Support research processes and incorporate new scientific findings into education programs.
- Independently develop and evaluate a personal continuing education and professional development plan.
- Provide orientation and in‑service training for new nursing staff.
- Refer individuals with abnormal screening results to appropriate primary care physicians or the HMH physician referral line.
- Identify socioeconomic conditions affecting health and make referrals to appropriate agencies, interpreting available services effectively.
- Administer social determinant of health surveys at events and make referrals via Now Pow as needed.
- Provide health promotion services in varied settings, including lifting, walking, bending, standing and stair climbing.
- Complete yearly department competencies and Health Stream modules on time.
- Perform other duties and/or projects as assigned.
- Adhere to HMH Organizational competencies and standards of behavior.
Qualifications
- Graduation from a School of Nursing approved by the NJ State Board of Nursing.
- Minimum 3–5 years of nursing experience.
- Willingness and ability to travel.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
Preferred
- Bachelor of Science in Nursing (BSN).
- Bilingual.
- Previous community health experience.
Licenses And Certifications Required
- Current valid NJ State Professional Registered Nurse’s license.
- Maintains a current NJ driver’s license.
- Annual CPR certification.
Compensation
Minimum Rate of $45.94 hourly.
