Center Well Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more. Our integrated care teams spend up to 50 percent more time with patients to deliver compassionate, personalized care and address factors that impact patient well-being.

Overview

Base pay range: $/yr - $/yr. The actual pay will be based on your skills and experience. Total compensation may exceed $300K depending on experience and location.

This position is located at our McAllen, Texas clinic. The role supports a value-based care model within a collaborative clinical environment.

Responsibilities
  • Evaluates and treats center patients in accordance with standards of care.
  • Monitors quality of care using data, chart reviews, and adherence to policy.
  • Coordinates patient services including referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Acts as an active participant and source of medical expertise with the care team through daily huddles.
  • Supports Regional Medical Director and Center Administrator in promoting professional, cooperative practice.
  • Completes medical record documentation in a timely manner, coordinating with a quality-based coder to optimize coding specificity.
  • Follows policies and protocols defined by Clinical Leadership; meets with RMD to review care quality, outcomes, and records issues.
  • Participates in opportunities for growth of existing or new services within the Center.
  • Participates in local primary care on-call program as needed.
  • Maintains licensing, certification, and accrediting body compliance.
  • Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks.
Qualifications
  • Current and unrestricted medical license or ability to obtain a medical license in Texas.
  • Graduate of an accredited MD or DO program.
  • Board Certification or eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (preferred).
  • Active and unrestricted DEA license.
  • Excellent verbal and written communication skills; strong interpersonal abilities.
  • Commitment to integrated team-based care and value-based practice settings.
  • Superior patient/customer service.
  • Basic computer skills, including email and EMR.
Preferred Qualifications
  • Medicare Provider Number and Medicaid Provider Number.
  • 2–5 years of directly applicable experience (preferred).
  • Experience managing Medicare Advantage panels and understanding of best practices in coordinated care within a value-based environment.
  • Knowledge of Medicare guidelines, coverage, and HEDIS quality indicators.
  • Bilingual English/Spanish (reading, writing, speaking).
