Medical Assistant (MA) – Chronic Care Management (CCM)
Giraldo Internal Medicine is seeking a motivated and compassionate Medical Assistant (MA) to support our Chronic Care Management (CCM) program. This role is ideal for an MA who enjoys patient interaction, care coordination, and helping patients manage chronic conditions such as diabetes, hypertension, heart disease, and other long-term illnesses.
The MA will work closely with providers and the care team to deliver high-quality, patient-centered care through both in-office and telephonic/virtual interactions.
Responsibilities- Room patients, obtain vital signs, and document medical histories
- Assist providers during exams and procedures
- Perform EKGs, phlebotomy, point-of-care testing, and other clinical tasks as trained
- Conduct outreach to patients enrolled in Chronic Care Management (CCM)
- Support monthly CCM follow-ups, medication reviews, and care plan updates
- Document CCM activities accurately in the Electronic Health Record (EHR)
- Educate patients on chronic disease management, medications, and preventive care
- Coordinate referrals, follow-ups, and communication with specialists
- Communicate lab results and provider instructions to patients
- Assist with scheduling, reminders, and telehealth support
- Maintain HIPAA compliance and clinic policies at all times
- Graduate of an accredited Medical Assistant program preferred
- MA certification (CMA, RMA, or NCMA) preferred
- Experience in primary care or internal medicine strongly preferred
- Experience with chronic disease management or care coordination is a plus
- Strong communication, organization, and documentation skills
- Comfortable using EHR systems and basic clinical equipment
- Bilingual (English/Spanish) required
