Overview
The purpose of this position is to perform detailed initial client assessments and nursing services, travel to clients' homes to complete assessments, assess accurately the clients’ living conditions and to obtain a clear understanding of the client’s level of function within his/her living environment. This position is also responsible for maintaining ongoing contact to monitor changes in the person’s condition and circumstances;
reassessing the person’s needs on a regular basis; termination; and advocating on behalf of the client to transition to another program. This position also completes face to face supervisory visits.
- Completes comprehensive initial client assessment; provides nursing services; conducts regular face-to-face supervisory visits, in client’s home, with the client and the client’s representative or caregiver (if needed); assesses and charts observations of the client’s condition at each visit; provides education to clients and families on proper home health care procedures and strategies.
- Develops standards and procedures for providing in-home services and evaluations; regularly evaluates, updates, and implements changes to standards and procedures.
- Coordinates with physicians, physical therapist, client’s representative, and other individuals in the client’s care; recommends tools or devices that may improve the quality of life for the client.
- Maintains files on all client’s initial assessment and nursing services; prepares progress notes describing events, reactions, situations, and follow-up that affects the client’s care; ensures all entries are signed and dated within one week of the occurrence.
- Monitors in-home services staff to ensure that quality homemaker and personal care services are provided; provides orientation, continuing education training, and guidance to in-home service staff to improve quality and quantity of service; plans and conducts educational programs for in-home services staff, to include eight continuing education hours annually for each staff after the first year of employment.
- Provides health education and screenings to senior members; assists with special projects (including evenings and weekends as necessary); attends and participates in ARC and DCH job-related meetings, training sessions, and seminars.
- Maintains a current, comprehensive knowledge and awareness of applicable laws, regulations, principles, and practices relating to the nursing field; reads professional literature; maintains professional affiliations; and attends meetings, workshops, hearings, and training sessions as required.
- Prepares monthly and annual reports to track clients, demographics, service costs, and reimbursements; processes a variety of documentation associated with department/division operations, within designated time frames, and per established procedures; receives and reviews various documentation; reviews, completes, processes, forwards, or retains as appropriate; prepares or completes various forms, reports, correspondence, and other documentation; compiles data for further processing or for use in preparation of department reports;
and maintains computerized and/or hardcopy records. - Operates a personal computer to enter, retrieve, review, or modify data, utilizing word processing, spreadsheet, database, presentation, Internet, e-mail, or other software; and operates general office or other equipment as necessary to complete essential functions.
- Communicates with supervisor, other County employees, court officials, attorneys, mental health care providers, doctors, clients, third party service contractors, the public, outside agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.
- Performs other related duties as assigned.
Requires a Bachelor’s of Science in nursing from a college or university accredited by the National League of Nursing; and one (1) year of related experience in a professional nursing role.
