Job Summary and Qualifications

The Clinical Pharmacist provides general clinical and operational pharmacy services, supervises technical staff, and ensures patient safety and quality of care.

Duties (Responsibilities)
  • Interpret and transcribe or verify orders accurately into the pharmacy computer system.
  • Accurately compound and dispense pharmaceuticals.
  • Comply with facility policy, procedures, and regulatory requirements.
  • Monitor drug therapy regimens for contraindications, drug‑drug, drug‑food, allergy and dosage appropriateness.
  • Suggest cost‑effective therapeutic alternatives to medical staff.
  • Maintain the hospital formulary and minimize nonformulary procurements.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO conversions, renal dosing and other Pharmacy Leadership–defined programs.
  • Assist with antimicrobial stewardship and other clinical programs.
  • Document all clinical activities and interventions accurately and completely.
  • Support quality improvement, risk management, and patient safety initiatives.
  • Complete notification reports of medication errors and adverse drug reactions.
  • Participate in training new pharmacists, pharmacy students, and technicians.
  • Maintain professional competency and relevant certifications (BLS/ACLS or PALS).
  • Provide technical support for automated dispensing machines and pharmacy information systems.
  • Supervise pharmacy operations and support the entire pharmacy team.
  • Intervene proactively in drug‑therapy disputes and collaborate with medical staff.
  • Participate in disaster, code, and other team drills as assigned.
  • Comply with regulatory standards and prioritize medication turnaround time policies.
  • Identify and implement efficiency improvements.
  • Act as supervisor in charge when necessary and participate in continuous quality improvement of pharmacy operations.
Qualifications
  • Active pharmacist license in good standing with the Texas State Board of Pharmacy.
  • Proactive prioritization of needs and effective resource management.
  • Excellent communication skills, both verbal and written.
  • Judgment and decision‑making based on job knowledge and experience, with consideration of impacted areas.
  • Customer‑orientation: builds trust and long‑term relationships with patients and staff.
  • Interpersonal skills and effective teamwork with diverse employees and external parties.
  • Proficiency in Microsoft Office, Meditech, purchasing/vendor applications and other pharmacy systems.
  • Knowledge of and adherence to organizational policies, procedures, and systems.
  • Basic mathematical, balancing, spelling, punctuation and transcription skills.
