QMHP- SUD Comprehensive Case Manager

Life Path Systems is seeking a QMHP- SUD Comprehensive Case Manager to report directly to the Program Administrator for SUD services and collaborate closely with the OSAR team, community resources, and contracted substance use treatment providers. The role serves individuals aged 13 and older seeking residential substance use treatment services.

Responsibilities
  • Assist eligible individuals by addressing personal needs, providing referrals, and linking to appropriate mental health, physical health, social, educational, housing, and other provider services.
  • Facilitate transportation, scheduling appointments, and medication pickup.
  • Maintain trauma‑informed, person‑centered care and conduct weekly contact with each individual from pre‑entry through treatment and post‑treatment.
  • Document all services, referrals, and follow‑ups in the CMBHS system as outlined by HHSC, completing documentation in a timely and accurate manner.
  • Track and report financial assistance and required measures per contract agreements.
  • Collaborate with the Program Administrator to ensure adherence to Health and Human Services Commission, Texas Administrative Code, SAMHSA grants, and Lifepath Systems policies.
  • Build and maintain professional relationships with community‑facing agencies and stakeholders.
  • Work independently and collaboratively to keep appropriate boundaries and promote better outcomes for individuals served, including crisis assessment and de‑escalation.
Specific Targets
  • Daily telephonic contact and at least one in‑person contact each week for residential treatment waitlist clients.
  • At least weekly contact with the individual and treatment team for residents in treatment.
  • Create discharge plans at least 5 days before discharge, including mental health, primary care, housing, and SDOH referrals.
  • Connect 95% of individuals to post‑treatment support within 1 week.
  • Complete direct services for at least half of work time; document within 48 hours of service.
Qualifications
  • Bachelor’s degree from an accredited college or university with major coursework in a human services field, qualifying as a Qualified Mental Health Professional (QMHP).
  • Preferred: 2 years of experience in substance use disorder treatment and with individuals with a mental health diagnosis.
  • English/Spanish bilingual preferred.
  • Reliable transportation and a valid driver’s license.

Location:

McKinney, TX. Salary: $45,000–$55,000 per year.
