Job Overview
Join to apply for the QMHP-CS Jail Diversion role at Life Path Systems
. Performs routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in a trauma-informed and person-driven care model to a caseload of individuals with legal issues.
- Provide person-directed case management to assist individuals in navigating access to medical, social, vocational, financial, educational, and other individualized services.
- Collaboratively complete uniform assessments, organize and document the plan for services to meet the individual’s identified needs.
- Use initiative and independent judgment in order to provide interventions in the community, home and school to individuals on caseload.
- Maintain timely and thorough documentation, manage clinic flow, adhere to crisis procedures, and assist with crisis coverage as needed.
- Work closely with criminal justice agencies to link criminal justice involved individuals with appropriate mental health and substance use disorder services in order to reduce recidivism while treating for serious mental illness to ensure clients receive the services they need to remain stable and functioning in the community.
- Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
- At least two years of related experience and/or training in the field of mental health preferred.
- Bilingual (Spanish/English) preferred.
- Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
- Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to write clearly and professionally.
- Ability to think clearly and logically to deal with potentially dangerous and/or emotional situations.
