Job Overview

The MCOT team member works under the direction of the Supervisor of MCOT. This position is team based to perform face‑to‑face crisis assessments, crisis intervention, crisis follow‑up and relapse prevention services for individuals with chronic mental illness within Collin County. Services are provided at the individual’s place of residence, schools, hospitals, jails and other safe community‑based locations 24/7, 365 days a year, including holidays.

Team members coordinate, monitor, provide crisis assessments, safety planning, advocate and serve as a liaison on behalf of the individual served, and follow up with individuals for a 90‑day period. Responsibilities include routine case management, skills training, psychosocial rehabilitation, community resource education, and discharge planning within a trauma‑informed and person‑driven care model. Team members may transport individuals in a center vehicle and are cross‑trained with the Crisis Center to perform assessment for crisis resolution and stabilization to reduce inpatient and law‑enforcement interventions.

Responsibilities
  • Perform face‑to‑face crisis assessments, interventions, follow‑up and relapse prevention services for individuals with chronic mental illness.
  • Coordinate, monitor, document assessments and develop recovery plans meeting identified needs.
  • Advocate, serve as liaison and support individuals, providing routine case management and skills training.
  • Travel and provide services at various community‑based locations on a 24/7 rotation, including weekends and holidays.
  • Transport individuals in a center vehicle and participate in cross‑training with the Crisis Center.
  • Ensure timely and thorough documentation, time management, organization, active collaboration with community partners, and strong teamwork.
  • Meet specific targets for assessment completion, documentation, follow‑up intervals (24h, Days 1‑7, 30, 60, 90), shift reports, chart reviews, and attendance at monthly team meetings.
Qualifications
  • Master’s degree and licensed as Texas LPC, LMFT, LCSW, or licensed psychologist; LPHA credential required.
  • Prior crisis experience preferred.
  • English/Spanish bilingual preferred; bilingual staff may translate for other staff and use approved language as needed.
  • Reliable transportation and valid driver’s license.
Knowledge, Skills, & Abilities
  • Knowledge of community resources, case management principles, objectives, standards, and methods; knowledge of program policies and procedures.
  • Ability to complete detailed, concise progress notes on all services provided, meeting timely documentation standards.
  • Experience providing rehabilitative services to individuals with mental illness.
  • Assess individual needs, coordinate services, communicate effectively, and train others.
  • Knowledge of the social services system and ability to connect individuals with appropriate services.
  • Clear, professional writing skills; strong verbal communication.
  • Logical thinking and de‑escalation techniques for potentially dangerous or emotional situations.
  • Time management, organizational skills, and ability to work independently and collaboratively.

Travel: 30% of travel expected to fit program needs, including transporting individuals, visiting community locations, and attending meetings or trainings at other center locations.
