Home Health FT LVN - Collin County at Ultimate Home Health Care

Ultimate Home Health is a privately owned Home Health Agency licensed by the State of Texas and Medicare certified. We provide Medicare‑certified home health care services throughout the DFW area, offering competitive pay rates and a flexible schedule.

• Provide direct nursing care according to the medical plan of treatment and nursing care plan.
• Utilize physical and psychosocial assessment skills; perform multi‑system assessments and identify current and potential care needs.
• Carry out physician orders for treatment and adhere to the visit frequency outlined in the plan of treatment.
• Organize and complete patient treatment tasks per established care plan.
• Implement and update care plan in collaboration with the Clinical Manager and Primary Care Clinician to reflect progress toward goals and outcomes.
• Coordinate care among team members and document in the clinical record per policy.
• Educate and train patients and caregivers on exercises and equipment needed to aid recovery.
• Provide education and instruction specific to patient and caregiver assessed needs, abilities, and readiness.
• Accurately document ongoing assessments, clinical findings, skilled care, and plan for next visit according to the established care plan.
• Be flexible with scheduling, including on‑call and department rotation for night and weekend coverage.

• Strong organization skills and time management.
• One year of previous geriatric home health experience required.
• Ability to multitask and meet timelines.
• Professional appearance and effective communication skills.
• Current Texas Registered Nurse (LVN) license.
• One year of clinical nursing experience.
• Current Texas driver’s license.
• CPR certification required.
• Experience with healthcare software (Kinnser preferred).
• One year of previous OASIS experience.
• Competency to carry out any skilled procedure that the physician orders within the scope of the Nurse Practice Act.
• PICC Line and Wound Vac experience (preferred).
