Human Edge Health is seeking an experienced Travel Occupational Therapist (OT) for a 13-week contract assignment in McMinnville, Oregon. This opportunity is based in a 24-hour nursing home setting with rehab services, offering a collaborative environment focused on functional independence, safety, and patient-centered care.

Assignment Details:

• Location: McMinnville, OR

• Facility: Nursing Home (Open 24 Hours)

• Start Date: ASAP

• Duration: 13 weeks

• Schedule: Monday–Friday

• Hours: 40 hours per week (5x8s)

• Setting: Skilled Nursing / Rehab

Responsibilities:

• Evaluate and treat patients with functional impairments impacting activities of daily living

• Develop individualized treatment plans to improve independence, mobility, and safety

• Collaborate with interdisciplinary teams including PT, SLP, nursing, and case management

• Document evaluations, progress notes, and care plan updates in accordance with facility standards

• Support rehabilitation goals to enhance quality of life and functional outcomes

Requirements:

• Active Oregon OT License (or ability to obtain prior to start)

• Minimum 1 year of OT experience; nursing home or rehab experience preferred

• Current BLS Certification (AHA)

• Strong clinical judgment, communication, and documentation skills
