Position: Community Support Coordinator for Vulnerable Populations
Become a Community Outreach and Client Support Coordinator to assist vulnerable populations in accessing essential resources. This role focuses on fostering stability and independence for individuals experiencing homelessness.

The ideal candidate will work to connect clients with housing providers, employment opportunities, and healthcare services. You will also provide support with job applications and identification processes while building positive relationships. Key Responsibilities:

• Connect clients with housing, healthcare, and employment resources

• Assist with resumes and job applications for clients

• Conduct regular follow-ups to monitor client progress

• Build strong relationships with community organizations and agencies

• Maintain accurate client files and prepare progress reports

Requirements:

• Excellent communication and organizational skills

• Strong computer proficiency including Microsoft Office
