• Develop and implement the nutritional component of programs offered through Cardiac Wellness Center and Heart Vascular Institute, including curriculum development and ongoing evaluation.
• Schedule, cancel, reschedule, and block appointments; provide one-on-one nutrition consultations; document visits in patient records; notify referring physicians as needed.
• Assess patient nutritional status using established standards; determine age-specific and disease-specific requirements; set nutrition goals with the patient; educate with materials tailored to age, disease, culture, readiness to change, and learning style.
• Collaborate with patients and health care teams; review medical records; reference the Nutrition Care Manual as needed; chart progress and outcomes; update nutrition prescriptions and weight goals as appropriate.
• Identify patients in group programs who need one-on-one counseling and ensure follow-up for those in one-on-one care.
• Attend team meetings and participate in ongoing program development, hospital/community education, and relevant nutrition/ wellness programs.
• Familiar with use of blood pressure cuffs and telemetry monitors; assist patients and clinical staff as needed; may record vital signs in patient charts.

• Certified as a Registered Dietitian by the American Dietetic Association (required).
• Bachelor’s degree in foods and nutrition and one to two years of supervised practice in foods and nutrition (required).
• Experience in health promotion and/or cardiac rehabilitation programs preferred.
• Knowledge of growth and development from adult through geriatric stages.
• Basic Life Support certification.

• Develop and implement the nutritional component of Cardiac Wellness Center and Heart Vascular Institute programs; ensure content is evidence-based and appropriate for the audience; review and modify content as needed.
• Manage scheduling of nutrition, clinical education, and exercise appointments; document visits in patient records; communicate with referring physicians as needed.
• Enter charges for program participation; participate in quality assurance and utilization review; use computer programs to track participation statistics and outcomes.
• Monitor program operations including equipment and supplies; assist in daily clinic operations such to scheduling and communication with patients and providers.

Part-time days; 3 eight-hour shifts per week (Mon–Fri, 8:00 AM – 4:30 PM). Schedule is not fixed and requires flexibility throughout the work week.
