Position: RN / Registered Nurse – Hospice

This is a full‑time hospice nursing position located in Mechanicsville, VA. The RN will provide compassionate, end‑of‑life care to patients with terminal illnesses across all four hospice branches, including Mechanicsville, Colonial Heights, Fredericksburg, and Williamsburg.
ScheduleFull Time – Monday to Friday, 8:30 AM to 5:00 PM. On‑call duties may be required as defined by the on‑call policy.
Office LocationMechanicsville office: 8149 Walnut Grove Road, Mechanicsville, VA 23111.
Coverage AreaTravel between all four hospice branches is required.
Responsibilities- Complete initial, comprehensive, and ongoing comprehensive assessments of patients and families to determine hospice needs.
- Provide a complete physical assessment and history of current and previous illnesses.
- Deliver professional nursing care utilizing all elements of the nursing process.
- Assess and evaluate patient status by:
- Writing and initiating plans of care.
- Regular re‑evaluation of patient and family/caregiver needs.
- Participating in plan revision as necessary.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop care plans that establish goals based on nursing diagnosis, incorporating palliative nursing actions.
- Include patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician.
- Counsel patients and families in meeting nursing and related needs.
- Provide health care instructions to patients as appropriate per assessment and plan.
- Assist patients with activities of daily living and facilitate self‑sufficiency and comfort care.
- Act as case manager when assigned, coordinating care for assigned caseload.
- Maintain accurate and relevant clinical notes regarding patient condition and care.
- Record pain/symptom management changes and outcomes.
- Communicate with the physician regarding patient needs and report changes; obtain and deliver orders as required.
- Coordinate care plans with community health professionals.
- Teach patients and families self‑care techniques.
- Provide medication, diet, and other instructions as ordered by the physician.
- Work in concert with the interdisciplinary group.
- Maintain a safe environment for the patient.
- Facilitate continuity of care with family, caregivers, and team members.
- Attend interdisciplinary group meetings.
- Ensure arrangements for equipment and other necessary items are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Engage in personal growth and development; maintain and upgrade professional knowledge through continuing education.
- Fulfill case assignments and participate in quality assessment and improvement activities.
- Graduate of an accredited school of Nursing.
- Current Registered Nurse licensure in the State of Virginia.
- American Heart Association CPR certification required; CPR may be completed upon hire.
- One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care.
- Bachelor’s degree preferred; one year of hospice experience preferred.
- Excellent observation, verbal and written communication, problem‑solving, and basic math skills.
- Licensed driver with an insured automobile in good working order.
