We are currently seeking a Licensed Social Worker (LSW) to join our dynamic team and contribute to our mission of delivering exceptional care and support to our clients and their families.

• Conduct comprehensive assessments of clients' social, emotional, and psychological needs.
• Develop, implement, and evaluate care plans in collaboration with a multidisciplinary team.
• Provide counseling and support to clients and their families, addressing social and emotional concerns.
• Liaise with community resources and external agencies to coordinate additional support services.
• Advocate for clients' rights and needs within the facility and in the broader community.
• Participate in team meetings and contribute to the development of policies and procedures related to social work and client care.

• Bachelor's or Master's degree in Social Work from an accredited institution.
• Valid Licensed Social Worker (LSW) certification.
• Demonstrated experience in a healthcare setting, preferably in a care facility or similar environment.
• Strong interpersonal and communication skills, with the ability to work effectively with diverse clients and teams.
