Advanced Practice Provider (Nurse Practitioner or Physician Assistant) – Pulmonology

Position Summary
Catholic Health Physician Partners seeks a compassionate and skilled APP to join our Pulmonology practice in Medford, NY, supporting Dr. Shahin Firoz Shaikh, MD, a respected specialist in Pulmonary and Critical Care Medicine. The APP will provide comprehensive pulmonary care to patients with acute and chronic respiratory conditions, collaborating closely with Dr. Shaikh and the care team to deliver patient-centered, high-quality care in both outpatient and, when applicable, hospital settings.
This role combines direct clinical care, patient education, and care coordination, helping to advance the mission of Catholic Health through excellence in service and compassion.
- Perform patient evaluations, including detailed histories, physical exams, and diagnostic assessments for patients with pulmonary disorders (e.g., asthma, COPD, interstitial lung disease, sleep apnea).
- Collaborate with Dr. Shaikh to develop and manage individualized treatment plans.
- Conduct follow-up visits, medication management, and monitoring of chronic conditions.
- Provide patient and family education regarding disease management, medication adherence, and lifestyle modifications.
- Order, interpret, and review diagnostic tests such as pulmonary function tests, imaging, and laboratory results.
- Support the practice with patient triage, care coordination, and follow-up of test results.
- Maintain accurate and timely documentation within the electronic health record (EPIC).
- Participate in quality improvement initiatives and contribute to best practices in pulmonary care.
- Assist with coordination of inpatient consultations or hospital follow-ups as needed.
- Master's degree in Nursing (NP) or Physician Assistant Studies required.
- Current New York State licensure as a Nurse Practitioner or Physician Assistant.
- National board certification (AANP, ANCC, or NCCPA).
- BLS and ACLS certifications required.
- Minimum of 2 years of clinical experience in Pulmonology, Internal Medicine, or Critical Care preferred; new graduates with strong interest in Pulmonology are encouraged to apply.
- Excellent communication, teamwork, and patient education skills.
