Position: Case Manager, Supported Housing
Full-Time/Part-Time: Full-Time
Location: Medford, NY
Salary: $55,000

Monday - Friday; 9am - 5pm
Description:
Concern Housing is seeking a dedicated Case Manager responsible for providing direct case management services to clients. The role involves being a resource familiar with community resources and services to assist clients in gaining access, as well as advocating for their needs.
Responsibilities include but are not limited to:
- Providing case management services such as service planning, medication monitoring, and assistance with clinical services, day programs, vocational services, community integration, and entitlements.
- Offering individual counseling and support services; responsible for documentation.
- Assisting with housing services, including maintenance issues.
- Conducting site visits as needed and directed by supervisor.
- Performing monthly fire safety inspections.
- Resolving landlord and lease issues, including lease negotiations and billing.
- Attending weekly supervision and staff meetings.
- Providing crisis intervention, including 24/7 cell phone coverage.
- Remaining current with all agency compliance documents and trainings.
Position Requirements:
Bachelor's Degree in Psychology, Rehabilitation, Social Work, or related mental health field, with at least 2 years of experience working in similar settings or with individuals with mental illnesses. Good verbal and written communication skills and sound judgment regarding client care are essential.
