About the Position

We are looking for Outpatient Peer Support Care Navigator to join our team at our Central Clinic in Medford, Oregon! The Outpatient Peer Support Care Navigator will utilize strategic self-disclosure with individuals to build connections and normalize mental health challenges. They will foster a comfortable, warm, and welcoming atmosphere for all clients in care (or who would benefit from engaging in care) with a person-centered lens, building quick rapport while demonstrating empathy and respect.

The Outpatient Peer Support Care Navigator will introduce Outpatient's integrated team-based care model to clients upon care entry, discuss benefits of this model, and address client questions and concerns. They will ensure all interactions are input into the EHR and schedule where appropriate documentation is completed to illustrate services provided and client progress. The Outpatient Peer Support Care Navigator may participate in community events to support client engagement strategies and foster community partnerships.

Schedule

Monday through Friday, 8:00am - 5:00pm (Full Time, Day)

Compensation

$20.00 - $24.00 per hour DOE/Credentials. Additional 5% Language Differential offered for Bilingual or Multilingual candidates (Spanish/English desired).

About the Program

Our Outpatient Program provides assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy, case management, skills training, and psychiatric (medication) services. The Psychiatric Services Team operates as a branch within the Outpatient Program, specializing in psychiatric, nursing, and medication services. Psychiatric services are provided both on-site and remotely through telemedicine. Columbia Care Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.

Qualifications

Successful candidates MUST:

  • Have personal experience with mental health challenges
  • Be Certified and Registered as a Peer Support Specialist (PSS) or Peer Wellness Specialist (PWS) through the Oregon Health Authority (OHA)
  • Drive and hold a valid driver's license from their state of residence (or obtain within 30 days of hire), maintain a safe driving record, and pass a DHS criminal background check
  • Be able to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, lift, carry, push, pull up to 40 pounds, occasionally use ladders, squat/kneel, and perform other physical tasks
  • Successfully complete crisis de-escalation and CPR trainings, including performing physical, hands‑on maneuvers and techniques and potentially lifting and transferring clients
