Job Title: Home Health Registered Nurse (RN)

Location: Medford, OR and Surrounding Areas

Duration: 13 Weeks (Contract)

Shift: Day Shift | 8-hour shifts | 8AM to 4:30 PM

Pay: $2586/week gross pay

• Provide skilled 1:1 home health nursing care to assigned patients

• Conduct up to 6 patient visits per day while meeting productivity standards

• Perform comprehensive assessments, medication administration, wound care, and patient education

• Complete accurate and timely documentation using EPIC/OASIS

• Coordinate care with physicians, case managers, and interdisciplinary teams

• Travel to patient homes within assigned geographic areas

• Minimum 1 year of recent skilled Home Health RN experience

• Proven OASIS documentation experience (must be clearly noted if absent)

• Ability to work independently in a home-based care environment

• Reliable personal transportation required

• Compliance with facility vaccination requirements (COVID primary series one booster)

• BLS (Required)

• Strong familiarity with EPIC charting system

• Experience managing geographically grouped patient caseloads

• Prior travel home health experience

• Local candidates accepted within a 50–75 mile radius of Medford, OR (locals accepted at the same rate)
