Nurse Practitioner​/physician Assistant Primary Care Clinic In Medford

Overview

Health Plus is seeking a full-time Nurse Practitioner or Physician Assistant to join our Primary Care team in Grants Medford, Oregon. The Nurse Practitioner operates as an established medical authority in a collaborative care team environment with physicians, and the Physician Assistant works under the supervision of the APP Executive Medical Director and designated Family Medicine Physician. Prepares and records patient's history, performs physical examinations and formulates treatment plans for patients.

Participates in management of patient's care by facilitating, expediting and coordinating routine diagnostic procedures, writing orders for appropriate medical care and determining needs for consultation. Triages patient calls and returns patient calls in conjunction with the physicians.

• Experience: Minimum 1 year of experience in a primary care setting required.
• Education: Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
• Licensure: Ability to obtain Nurse Practitioner (NP) license by the Oregon State Board of Nursing (OSBN) or Physician Assistant (PA-C) certified by the National Commission on Certification of Physician Assistants (NCCPA) and licensed by the Oregon Board of Medical Examiners (OBME) required. Must be eligible as a provider for Medicare, Medicaid, and other federal health programs. Provider shall meet all qualifications to participate in hospital's programs for professional malpractice and other liability coverage.
