Job Opportunity – Case Manager RN – Oregon

Start Date: February 16, 2026
Location: Oregon

Schedule: 8-hour shifts preferred (10-hour shifts may be considered); weekend coverage varies by facility need; 40-hour orientation provided

Highlights:

• Work in a 108-bed short-term acute care facility with daily census of 95–110 patients
• Epic charting system experience required
• Expected case ratio of 1:15; weekend assignments focus on discharges and new assessments only
• Comprehensive interdisciplinary support including IV teams, physical therapy, respiratory services, pharmacy, and social services
• Case Management Assistants available to support with SNF authorization, transport coordination, DME dispensing, and placement referrals
• Float coverage across multiple acute departments: ICU, medical, med-surg, telemetry, observation unit, and ED

Requirements:

• Active RN license in Oregon (or ability to obtain prior to start)
• Minimum 1 year of RN experience
• Epic charting system experience required
• Current BLS certification
• Experience with admission criteria, appeals and denials, care coordination, discharge planning, medical necessity, and CMS guidelines
• Must have reliable personal vehicle for this assignment
• Candidates must reside more than 50 miles from facility
• Two supervisory references covering at least 1 year within the last 3 years
