American Traveler is hiring a Home Health LPN with at least 1 year of skilled home health experience and HCHB proficiency for a day-shift assignment in Southern Oregon.
Requirements- Active Oregon LPN/LVN license required before start date; temporary licenses not accepted (license required if applicable)
- Minimum 1 year of skilled Home Health experience required
- HCHB (Homecare Homebase) proficiency required
- OASIS experience required for staff completing Start of Care (SOC) visits
- BLS certification required
- Must have reliable personal transportation and an active driver's license
- Full 7-year work history with all gaps explained required for consideration
- Two professional references from a supervisor within the past 2 years required for consideration
- Skilled Home Health setting with a 1:1 patient ratio
- Approximately 30 productivity points per week, up to 6 patients per day
- Day shift schedule, 8-hour shifts (8:00 AM-4:30 PM)
- Weekend support may be required
- No on-call requirement, though on-call rotation may be required per facility discussion
- Documentation completed using Home Care Home Base (HCHB), Remote Client laptop, and Rover phone application
- Visits are grouped by the facility to minimize driving, though service area may extend up to 60 miles or 60-90 minutes of total daily driving
- Business casual attire required; no jeans; scrubs optional with no specific color requirement
- Free parking available on site
- Mileage reimbursed at the current IRS standard rate
- Up to 3 days of RTO may be automatically approved; blackout periods apply during Thanksgiving, Christmas, and New Year's weeks
- All time off requests must be submitted at the time of application
- Monday orientation required, including completion of billable training modules
- Hospice/Home Health checklist completion is mandatory
- Travel pairs accepted, but each traveler is assigned their own territory and must have personal transportation
- Local candidates accepted within a 50-75 mile radius at the same rate
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
