Opportunity:

PT - Physical Therapist

Job Details:

• 13-week contract with guaranteed 40 hours per week

• Monday through Friday, 8:00 AM to 5:30 PM schedule

• Occasional travel required to central/Northern regions with mileage reimbursement at government-approved rate

• Rotate one Saturday every 6-8 weeks (receive weekday off when working Saturday)

• No holiday requirements

• Can be low censused and placed on call with 2-hour minimum callback pay

• Contract eligible for 36 hours of unbillable low census hours

• Master's Degree in Physical Therapy required

• Home health experience with adult/geriatric population required

• Experience with orthopedic, neurological, and functional mobility diagnoses

• Wisconsin Physical Therapy license and NPI number required

• AHA BLS certification required

• Must live outside 50-mile radius from facility

• Average productivity expectation of 4.5 visits per day

• Point of service documentation using Net Smart EMR on provided laptop

• Cell phone provided for GPS and communication

• Any scrubs or business casual attire acceptable

• Evaluate, treat, and plan patient care according to Physical Therapy standards

• Document to meet regulatory and reimbursement requirements
