Key Responsibilities:

• Conduct resident assessments and maintain care documentation

• Implement and adjust care plans according to resident needs

• Participate in care conferences and collaborate with team members

• Educate residents and families about their healthcare plans

• Respond to medical emergencies and manage changes in conditions

Requirements:

• Registration as a Licensed Practical Nurse (LPN)

• Experience in senior or long-term care preferred

• Strong clinical and communication skills

• Current CPR and First Aid certification

• Proficient in electronic documentation processes
