As a Disability Services Worker 2, you will provide essential support to individuals with developmental disabilities.

Your role includes developing Individual Support Plans, promoting personal growth, and ensuring individuals' health and well-being. You'll collaborate closely with families and stakeholders to foster meaningful participation in various community activities.

Key Responsibilities:

• Provide personalized support with daily living and personal care

• Develop and monitor Individual Support Plans with stakeholders

• Promote self-advocacy and community inclusion opportunities

• Monitor health and coordinate medical follow-ups

• Maintain accurate documentation and reports

Requirements:

• Diploma or certification in Rehabilitation or related field

• Strong communication and problem-solving skills

• Knowledge of developmental disabilities

• Flexibility in scheduling and teamwork

• Valid driver’s license and satisfactory Police Information Check
