Delta Care Rx and Dragonfly Health are seeking an experienced pharmacy professional to support our hospice medication services in Medina, OH.

• Check all patient medication orders for suitability and enter orders into the system for the inpatient unit.
• Oversee the dispensing, ordering, and restocking of medications for the hospice unit.
• Confirm and review deliveries from shipping vendors and pharmacy couriers before they are shipped.
• Ensure compliance with all pharmacy laws and regulations.
• Participate in pharmacy operations and inventory management.
• Help maintain excellent customer service and client relations.
• Create and update training documents for new employees.
• Manage inventory according to financial guidelines and oversee ordering supplies, including for hospice units.
• Assist the Pharmacy Manager with regular reviews of the formulary and inventory for inpatient facilities.
• Operate the closed‑door hospice pharmacy in Medina, OH—open 7 days per week with a weekend rotation.

• Active state pharmacy license required; additional licenses may be needed.
• LTC dispensing pharmacy experience.
• Preferable hospice/palliative care and multidisciplinary team leadership experience.
• Skilled in pain medication dosing, generic substitutions, formulary management, and prior authorizations.
• Detail‑oriented with strong project management abilities.
• Ability to maintain confidentiality and exercise good judgment.
• Strong typing, computer, verbal, and written communication skills.
• Excellent interpersonal and customer service abilities.
• Familiarity with Framework LTC dispensing software and pharmacy automation preferred.
