Provide comprehensive inpatient hospital care
Coordinate care with multidisciplinary teams

Focus on reducing readmissions through proactive patient management and communication
Deliver high-quality, patient-centered outcomes
Qualifications
Board Certified in Internal Medicine or Family Medicine
Strong communicator and collaborator
Commitment to quality, safety, and continuity of care
