Job Description
Join our team as a Physical Therapist in an Assisted Living Facility (ALF) with occasional home visits. You’ll autonomously make clinical decisions, manage a full caseload, and collaboratively support patient care.

Key Responsibilities

• Manage full caseload of patients with individualized treatment plans
• Maintain flexible schedule to meet patient needs
• Communicate with patient families and community staff
• Ensure guaranteed hours and job security
• Exercise clinical autonomy with support from experienced therapists
• Work primarily in ALF setting with occasional community work
• Create schedule and provide continuity of care
