Medicare Home Health Registered Nurse – Per Diem (Florida, Melbourne)

Job Responsibilities

• Performs complete assessment/evaluation including home safety, pain, nutrition, fall risk and DME needs.
• Develops and implements a Plan of Care based on assessment findings and physician orders.
• Complies with State and Federal Regulations and JCAHO standards for home care services.
• Provides case management for all disciplines.
• Supervises Home Health Aide every 14 days and the Licensed Practical Nurse.
• Assists weekend staff for coverage to maintain continuum of care.
• Participates in all aspects of Telehealth.
• Documents alterations in the Plan of Care with treatment orders sent for physician signature on the day of change and progress toward goals.
• Rotates On Call.
• Evaluates patient outcomes, communicates progress and changes in condition to physician.
• Informs patient/family of visit schedule in advance by calling the night before and/or if schedule changes.
• Ensures appropriate utilization of visits and services with documentation completed within two days of the visit.

Qualifications

• Graduate of an accredited school of Professional Nursing.
• Minimum of 1‑year acute care experience as an RN; home care experience preferred.
• IV Therapy and phlebotomy experience preferred.
• Criminal history screening.
• Computer skills.
• Current Florida driver’s license, automobile insurance, and reliable transportation.
• Must hold current AHA BLS Healthcare Provider Completion Card before start date and maintain.
• Current licensure in the State of Florida as a Registered Nurse.
