The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home.

Responsibilities include:

• Providing services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Completing and submitting daily progress notes for each visit.
• Observing and reporting the patient's nursing needs and instructing family members in patient care and maintaining a healthy environment.
• Monitoring, recording, and reporting patient responses to treatment and any changes in condition to the supervising nurse or physician.

• Current LPN licensure in Florida
• Valid Driver's License, vehicle insurance, and access to reliable transportation
• CPR Certification
