Wound Care Nurse; RN​/LPN

Location: Melbourne, FL
Schedule: Every other weekend | 7:00 AM–7:00 PM
Pay Rate: $27–$40 per hour, based on experience

Melbourne Healthcare and Rehabilitation is seeking a skilled and compassionate Wound Care Nurse (RN/LPN) to join our team part-time. In this role, you will collaborate with the interdisciplinary team to provide individualized wound and skin care that promotes healing, comfort, and quality of life for our residents.

• Collaborate with the interdisciplinary team to develop, implement, and evaluate individualized wound care plans.
• Provide wound treatments and monitor healing progress within your scope of practice and in accordance with provider orders.
• Perform skin checks, document findings, and promptly communicate changes in residents’ conditions.
• Identify residents at risk for pressure injuries and implement preventive measures.
• Monitor discomfort associated with wound care and assist with prescribed pain management measures.
• Contribute to applicable Minimum Data Set (MDS) sections and maintain accurate, timely documentation.
• Follow facility wound care policies, infection prevention practices, and established treatment protocols.
• Educate residents and families about wound care treatments and preventive skin care.
• Collaborate with nursing and dietary staff to address nutritional needs that affect wound healing.
  
  Communicate recommendations for dressings, treatments, and pressure-relieving devices to the appropriate clinical team members.
• Promptly report suspected abuse, neglect, and resident safety concerns in accordance with facility policies.

• Active, unencumbered RN or LPN license in Florida.
• Minimum of two years of nursing experience.
• Wound care experience preferred.
• Skilled nursing or long-term care experience is a plus.
• Strong observation, documentation, communication, and organizational skills.
• Compassionate, dependable, and committed to resident-centered care.
