Community Nurse; Band 5; Home Care Impact
HCRG Care Group is seeking a Community Nurse (Band
5) to join the Melksham team, delivering high-quality, patient-centred care in patients' homes. You will work with GPs and multidisciplinary teams to assess needs, plan and deliver care, promoting independence and positive health outcomes.
You will be NMC registered, possess a full UK driving licence and be proficient with IT systems. The role offers 37.5 hours per week at Melksham Community Hospital with AFC terms and NHS Pension.
