We have an exciting opportunity for experienced Registered Nurses to join our recently formed Integrated Discharge Team.
Role OverviewThe Nurse Flow Coordinator role is crucial for managing and facilitating patients' safe and timely transitions from hospital to their next destination, such as home or another facility.
Key Responsibilities- Coordinate multidisciplinary teams (doctors, nurses, social care, third sector partners).
- Identify and resolve potential barriers to discharge.
- Ensure patients have necessary equipment and support.
- Maintain accurate records and liaise with families and external services.
- Support patient flow and contribute to a positive hospital experience.
- Full UK NMC registration.
- Ability to work without direct supervision and across multidisciplinary teams.
- Flexible to work weekdays and weekends on a rotational basis as per service needs.
