Tufts Medicine is looking for a dedicated inpatient clinical pharmacist to support Med/Surg services. The pharmacist will provide comprehensive pharmaceutical care, review medication orders, collaborate with healthcare teams, and lead quality improvement initiatives within the hospital setting.
Job Profile SummaryThis role focuses on delivering pharmacy services in a hospital setting, dispensing medicines (incl. controlled substances), and improving processes and policies through advanced pharmacy knowledge. The pharmacist may oversee the work of other professionals, direct lower‑level staff, and lead system design and implementation.
Job OverviewClinical pharmacists work closely with physicians, nurses, and other health practitioners to review and approve medication orders, monitor drug therapies, and provide drug information. The position enhances patient outcomes and serves as a resource for clinical staff, educators, and researchers.
Job DescriptionMinimum Qualifications
- Bachelor’s degree in Pharmacy
- Pharmacist License
- One (1) year of related experience
- Doctor of Pharmacy (Pharm
D) - Board Certification
- Four (4) years of related experience
- Clinical pharmacy expertise with direct patient care experience (via residency or prior job)
- Experience in drug utilization review and analysis
- Direct and coordinate clinical activities of the assigned pharmacy service area.
- Prepare, compound, and dispense medications in accordance with state and federal laws.
- Review physician orders for appropriateness and potential interactions.
- Contact physicians for clarifications and interventions as needed.
- Ensure accurate medication preparation, labeling, and compliance with regulations.
- Provide accurate, timely drug information to staff, patients, and families.
- Conduct medication history interviews, reconcile therapies, and educate patients.
- Accurately anticipate controlled drug needs and complete necessary records.
- Complete medication station inspections and ensure quality control.
- Participate in medication education and documentation in patient progress notes.
- Utilize Pharmacy computer system for order entry and necessary data input.
- Participate in multidisciplinary team meetings and medication reconciliation processes.
- Conduct home visits for identified patient populations.
- Engage in Pharmacy Quality Programs across operational, clinical, and financial areas.
- Collaborate with pharmacists for operational support (including evening, weekend, and on‑call coverage).
- Provide continuing education, precept, and mentorship to clinical staff.
- Participate in development of treatment guidelines and clinical pathways.
- Contribute to departmental initiatives and strategic planning.
- Frequent standing, walking, and lifting of 5–10 lbs.
- Occasional light lifting (boxes, equipment) and stooping or kneeling.
- Manual dexterity for prescription medication handling and computer use.
- Visual acuity for medications, screens, and reports.
- Knowledge of pharmacy laws, regulations, and professional standards.
- Superior verbal and written presentation skills.
- Strong communication and presentation abilities.
- Familiarity with integrated health networks and formulary management.
- Project and resource management skills.
- Creative problem‑solving and critical thinking.
- Understanding of confidentiality standards for patient information.
- Knowledge of clinical best practices for medication use optimization.
- Proficiency in Microsoft Office (Access, Excel, PowerPoint, Word).

- Proficiency in Motivational Interviewing techniques.
- Prioritization and efficiency in resolving critical issues.

- Ability to work independently with minimal supervision and as part of a team.
