You will evaluate patients at home, design personalized care plans, and deliver interventions to improve strength, balance, mobility, and safety, with documentation via a mobile EMR.
Flexible scheduling, mileage reimbursement, and a supportive team are offered; MA license, DPT/MPT, CPR, and reliable vehicle are required.
