Job Description Leads, guides, and coordinates staff, students, and support staff in the performance of duties. Provides social work services to patients and/or their families throughout the continuum of care. Performs all functions of Social Worker.

• Assists in the formulation and development of departmental and interdepartmental programs, policies and procedures.
• Contributes to the assessment of social health needs in the facility and community; participates in the development of effective health and welfare services/resources.
• Collaborates with other staff members to identify unmet psychosocial needs of patients.
• Participates in appropriate research, staff development programs, and community liaison activities.
• Contributes to educational activities including presentation, training of graduate students, attending/championing workshops and seminars.
• May participate in hospital committees to discuss social work practice and procedures.

• Master of Social Work required.
• Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
• 3-5 years of relevant experience, required.
