** Responsibilities
* ** Provide skilled nursing care and administer prescribed medications and treatments.
* Perform G-tube/PEG care, tracheostomy care, and suctioning as required.
* Monitor patient conditions and report changes promptly.
* Provide ventilator support when required and appropriately trained.
* Educate patients and caregivers and maintain accurate documentation.  
** Qualifications
* ** Active New York State RN license.
* Minimum of 6 months of home care experience.
* Current CPR/BLS certification.
* Strong clinical and communication skills.
* Compassionate, reliable, and able to work independently.
* Ability to meet physical requirements, including lifting up to 50 pounds as needed.
