Transition Specialist - Evening Reporting Center - PART-TIME

Job Description Provides and builds a sustainable model for career exploration and readiness for juvenile justice youth. Evaluates participants’ needs, prepares and implements plans to address behavioral, social, psychological, and career readiness issues. Offers individual and group counseling and consultations with teachers and other professionals. Maintains accurate case records.
In consultation with school-based and community partners, identifies student cohorts for project participation. Develops and distributes interest surveys, performs academic and behavioral reviews, and develops holistic career readiness plans.Facilitates implementation and compliance of support and career readiness services. Monitors the integrity of interventions related to contract goals, including progress reviews and assessments.Provides parent and teacher consultations, holds conferences, and reviews student progress in various areas.Develops and facilitates the transition process from juvenile facilities to re-entry into academic and career education settings. Creates transition plans and coordinates school enrollment.Serves as an advocate and liaison between schools and agencies providing services to juvenile justice students. Manages meetings, monitors agreements, and handles correspondence.Collaborates with social, community, mental health, and workforce programs. Advocates for community and school services to meet student and family needs.Facilitates grant-specific activities and provides individual/group interventions. Arranges projects and extended learning opportunities.Gathers data for reports on program activities, progress, and evaluations.Conducts staff and partner meetings to ensure program goals are met.Assesses program effectiveness and recommends improvements.Performs other duties as assigned.Qualifications
Graduation from an accredited college with an Associate’s Degree in Social Science, Education, or related field plus two years of experience working with at-risk youth or juvenile justice; total of four years of education/experience. Valid driver’s license required. Bachelor’s degree preferred. Degree equivalency:
Bachelor’s = 4 years + experience;
Master’s = 6 years + experience.
