Responsibilities
Lakeside Behavioral Health System invites applications for a Triage and Assessment Counselor/Nurse. The role includes telephonic and in‑person risk assessment and crisis intervention, initial contact with referral sources and potential patients, determination of appropriate level of care, and maintenance of communication with referral sources, families, patients and unit staff. Walk‑ins are received with compassion, empathy, a sense of urgency, and a commitment to an appropriate assessment.
Accurate documentation is a critical component of the job.
The Triage and Assessment Counselor/Nurse provides risk assessment and crisis intervention for psychiatric patients ranging from children to seniors, ensuring each person receives an appropriate assessment and continuous care. The position requires close collaboration with psychiatric teams, referral sources, and family members while maintaining thorough and accurate documentation.

Education: Licensed Physicians, APRN’s, Licensed Psychologist, Licensed Social Workers, Licensed Professional Counselors, Registered Nurses, Masters in Social Work, Masters in Counseling, Masters in Marriage and Family Therapy, or Masters of Psychology required.
Experience: Prior emergency department, intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting, but not required.
Licensure/Certification: Licensure and registration by the Tennessee Board of Nurse Examiners as a registered nurse.
Knowledge: Must possess knowledge of general and psychiatric nursing processes, therapeutic relationships and processes, age‑specific growth and development, limit setting, crisis and behavior management, and CPR training.
