Shift Type: Night Shift (2:00 pm–10:00 am)

- Communicate effectively, both verbally and in writing, ensuring clarity to patients and team members
- Coordinate with the healthcare team to assess, plan, implement, and evaluate patient care plans
- Administer medications orally, subcutaneously, IV, and record them accurately; report adverse reactions per policy
- Record vital signs and patient medical information
- Monitor and adjust specialized equipment, interpret and record electronic displays
Initiate corrective action when patients show adverse symptoms - Provide bedside care, change dressings, insert catheters, start IVs, prepare equipment, and aid physicians
- Respond to life‑saving situations in line with nursing standards and protocols
- Participate in discharge planning
- Manage time efficiently and use organizational skills to meet commitments
- Document all care information concisely, accurately, and timely on appropriate forms
- Perform other related duties as assigned
- Current LPN or RN license in the state of qualification
- Up‑to‑date first aid and CPR certification
- At least one year of experience as an LPN or RN in a long‑term care setting (preferred)
- Knowledge of Matrixcare preferred
