We seek a Nurse Practitioner or Physician Assistant to provide on-site primary care in Assisted Living and Memory Care communities across Minnesota, with a Monday–Thursday schedule and an optional Friday.
You’ll manage about 200 patients and coordinate with LPNs, RNs and social workers.
