Job SummaryThis position is responsible for facilitating patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital, and Froedtert Community Hospitals. The individual will attend emergency and non-emergency transfer requests and coordinate all transfer requests promptly and courteously. The role collects pertinent clinical information from the sending provider – including the patient’s diagnoses, diagnostic test results, current treatment plan, past medical/surgical history, and the reason for the transfer.
Together with the accepting physician and the sending provider, the individual also responds appropriately using discretion and independent judgment to determine the appropriate level of care/location, transport mode (air/ground), and the type of unit. The worker must have in-depth knowledge of EMTALA regulations for ED transfers and act as a resource to key stakeholders. This individual must effectively prioritize the transfer based on patient acuity and clinical information presented and follow established hospital workflows and protocols, often in difficult or critical situations.
Problem‑solving and decision‑making skills are required in a fast‑paced environment. All discussion pertaining to the transfer request is documented in EPIC as part of the transfer documentation. The worker collaborates continuously with physicians, nursing, admitting, and patient‑flow staff to ensure timely patient transfers and optimize occupancy across hospitals. The individual also facilitates consult requests from outside‑facility providers with Froedtert & MCW physicians, participates in conference calls, documents for the provider’s behalf, and attends to transfer needs that arise from the consultation.
When working the 0000‑0800 shift, the individual is cross‑trained to assist with patient phone triage when needed.
A minimum of four years of nursing experience, with at least three years in acute‑care nursing, is required. Previous critical care or ED experience as an RN is preferred.
Education DescriptionBachelor’s degree in Nursing is required. In lieu of a Bachelor’s degree, either an additional three years of experience or active enrollment in a Bachelor’s program with a defined end date is required.
License DescriptionRequires current state of Wisconsin registered nurse license or a multi‑state nursing license from a participating state in the Nurse Licensure Compact (NLC).
