RN TRANSFER COORDINATOR

US-WI-MENOMONEE FALLS at our WOODLAND PRIME 400 facility.
Job type:
Full-time, 0.75 FTE, 30 hrs/week. Shift 3 (Wed, Thurs, Fri, Sat, Sun 22:00-08:00; B week Wed & Thurs 20:00-06:00; holidays per schedule).
This position is responsible for facilitating patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital, and Froedtert Community Hospitals. The individual will attend emergency and non‑emergency transfer requests and coordinate all transfer requests promptly and courteously. Collect pertinent clinical information from sending provider, including patient diagnosis/es, diagnostic test results, current treatment plan, past medical/surgical history, and the reason for the transfer.
Working with the accepting physician and the sending provider, the individual will respond appropriately using discretion and independent judgment to a wide variety of requests to determine appropriate level of care/location selection, transport mode (air/ground) and type of unit. Comprehensive knowledge of the EMTALA regulation for ED transfers and serving as a resource to key stakeholders. Ability to prioritize the transfer based on patient acuity and clinical information and follow established hospital workflows and protocols.
Frequently must participate in difficult or critical situations. The individual must employ problem solving and decision‑making skills in a fast‑paced environment. All discussions regarding the transfer request are documented in EPIC as part of the transfer documentation. Constant collaboration with physicians, nursing, admitting, and hospital patient flow is required to ensure timely patient transfers, meeting hospital standards. Work closely with hospital patient flow to strategize patient placement and optimize occupancy across hospitals.
Cooperate and communicate effectively with team members to accomplish workload. The individual will also facilitate consult requests from outside facility providers with Froedtert & MCW physicians, participate in conference calls, document for the provider, and attend to transfer needs arising from consultation. During shifts between 00:00-08:00, the individual is also cross‑trained to assist with patient phone triage when needed.
A minimum of 4 years of nursing experience, at least 3 years in acute care nursing. Previous critical care or ED experience as an RN is preferred.
EducationBachelor’s in Nursing required. If no Bachelor’s degree, either an additional 3 years of experience or active enrollment in a Bachelor’s program with a defined end date is required.
LicensureRequires current state of Wisconsin Registered Nurse License or a multi‑state Nursing License from a participating state in the NLC.
