Location:
Menomonee Falls (Woodland Prime 400). Hybrid work arrangement with at least 8–12 months onsite before transitioning to hybrid.
FTE: 0.90 – Standard
Hours:
36.00 – Shift: Shift 2 (Weekdays 1400‑2230, every 3rd weekend, up to 3 holidays annually with shift and weekend differentials).
This position facilitates patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West HGand and Froedtert Community Hospitals. The coordinator attends emergency and non‑emergency transfer requests, gathers pertinent clinical information, determines appropriate level of care, selects transport mode (air/ground), and optimizes unit placement. The role requires advanced knowledge of EMTALA, effective prioritization, problem‑solving in fast‑paced environments, and documentation in EPIC.
The coordinator collaborates with physicians, nursing, admitting, and patient flow teams to ensure timely and efficient transfers.
Minimum 4 years of nursing experience with at least 3 years in acute care. Preference for previous critical care or ED experience.
EducationBachelor’s degree in Nursing required. If lacking a bachelor’s, an additional 3 years of experience or active enrollment in a bachelor’s program with a defined end date is required.
LicensureCurrent Wisconsin Registered Nurse license or a Multi‑state Nursing License via the Nurse Licensure Compact.
