• Complete initial and ongoing assessments
• Handle admission of residents
• Medication Monitoring
• Health monitoring/screening
• Monitor diabetic blood sugars
• Diabetic cares as needed, i.e. fingernail and toenail care
• Checking lab test report sheets from Clinic,follow through with medication changes as needed
• Complete andreview progress notes
• Develop and update care plans
• Arrange doctor appointments and setting up transportation
• Complete all nursing procedures
• Audit Charts
• Schedule and supervise all staff in absence of director
• Monitor TB testsfor residents and staff
• Be prepared to work the floor if needed
• Usethe appropriate lift per residents’ care plans
• Communicate to Director what follow-up will beneeded over the week
• Perform additional duties as assigned
• On Call responsibilities

• Lift/carryup to 25 pounds occasionally
• Push/pull medication carts up to 300 pounds occasionally
• Twist,stoop, bend, squat, kneel, reach, walk, and sit frequently
• Ableto communicate orally and/or in writing with residents, co-workers, families,and the public

• Valid Wisconsin Registered Nurse license
• Three to five years nursing experience with a minimum of 2 years in long term care
• Must possess leadership qualities and excellent communication skills
• Supervisory experience preferred
• BSN preferred
• Follow/adhere to organization,federal/state regulations and guidelines, including resident/client rights, standard precautions, fire safety, and first aid
