The Patient Navigator plays a critical role in supporting health center patients throughout their care journey by coordinating diagnostic testing and ensuring seamless follow-up with outside entities. This position focuses on obtaining diagnostic results and closing the loop on patient referrals by proactively managing the external work queue, identifying referrals requiring attention, and facilitating communication between patients, providers, and external healthcare facilities.

The Patient Navigator acts as a liaison to support both patients and the health center team, promoting timely access to services and adherence to recommended follow-up care.

Key responsibilities include outreaching patients to schedule necessary appointments, assisting in overcoming barriers to care such as transportation or financial challenges, and retrieving diagnostic and referral results to ensure comprehensive documentation and continuity of care. Monitoring, updating, and following up on patient referrals using external referral management systems to ensure patients are scheduled with outside specialists, services, or community partners. Addressing patient navigation needs flagged within the internal electronic work queues, providing a high level of support and guidance to patients throughout the care continuum.

Identifying and reducing barriers to care, providing support directly or connecting them to community-based resources.

Position Requirements:

  • High School Diploma or GED Equivalent
  • Associate's or Bachelor's Degree (preferred). Graduate of an accredited school of Practical Nursing preferred

Experience:

  • Experience in healthcare, health center or clinical case management (preferred)

Licensure/

Certifications:

  • MA, LPN, RN (preferred)
  • BLS certification in the first 6 months (employer provided)
